Travel Nurse RN - Case Manager - $2,275 per week in Niagara Falls, NY - Weekly Pay Range - $2048 To $2276 Position Summary: * Care for ill, injured, or convalescing patients or persons with disabilities in hospitals, nursing homes, clinics, private homes, group homes, and similar institutions. * May work under the supervision of a registered nurse. Licensing required. Reports To: * Contracted Medical Facility’s onsite Nurse Manager/Supervisor, Physicians * Cynet Health Account Manager * Cynet Health Director of Nursing Duties and Responsibilities: * Performs admission, concurrent, and retrospective reviews on all patients. * Ensures that status and level of care determination are accurate upon admission. * Ensures that pre certifications/authorizations are obtained and documented appropriately for all admissions, surgeries, and procedures. * Follows Medicare guidelines regarding utilization review. * Submits telephonic and/or fax notification/clinical information * Follows up with insurer for authorizations. * Identifies potentially high risk patients and refer to Social Work. * Provides coaching to physicians as needed regarding effective clinical documentation Conducts Initial Assessments to develop plan of care. * Ensures that treatment plan is implemented, re-evaluated, and updated daily * Facilitates effective communication between members of the treatment team * Coordinates meetings w/ families and treatment team * Facilitates arrangement of post-acute care services * Acts as an advocate for the patient/family * Follows Medicare guidelines regarding patient choice, admit/discharge notifications, and appeals. * Maintains accountability for facilitating clinical patient progression through a defined plan of care to achieve optimal outcomes. * Identifies, document, and report quality, cost, and risk variances related to patient’s plan of care. Minimum Job Requirements: * Graduate of an accredited school of nursing, with current licensure in state practicing in * good standing in the state of practice. * Evidence of minimum two years of current hospital experience. * Evidence of current BLS credential mandatory.

Travel Nurse RN - Case Manager in White Plains, NY - Shift: M-F 9A-5P with a weekend rotation ( Sat & Sun) Sign On Bonus : 5000 Description: The RN Care Manager performs the initial comprehensive assessment on admission in accordance with the Care Management Department policy, screening all patients by utilizing established tools for high risk indicators (i.e. AIMS, Geisinger, polypharmacy) to ensure high risk patient populations receive the appropriate supportive services for discharge to prevent readmission and assess all populations for potential discharge planning needs. Performs all transitional planning activities including but not limited to: Patient/caregiver education Arrangement of all aftercare Commercial payer involvement High utilizer patient root cause Communicates and collaborates all relevant patient information to appropriate health team members. Ensures department goals of LOS reduction, readmission prevention, denial reduction, and satisfaction improvement are realized. Documents per department requirements. Participates in Performance Improvement activities as needed. Performs all UR activities per department policy: Commercial payer reviews Disposition determination, Retro/self-audit MC variance Admission clinical review. Performs all other related duties as assigned. Education & Experience Requirements BSN or related Bachelor's degree required 5 years clinical ED, critical care or med/surg experience required Previous case management experience preferred Knowledge of healthcare financial, regulatory, and payer issues preferred Knowledge of state, local, and federal programs strongly preferred Experience in use of Milliman/InterQual criteria Certification/License: Registered Nurse (RN) NYS license required PRI/Screen certification strongly preferred. C.C.M. (Certified Case Manager) certification preferred Computer skills essential
