Travel Nurse RN - Home Health - $1,542 to $1,699 per week in Portsmouth, VA - Job Description & Requirements Registered Nurse – Home Health - Travel - (HH RN) StartDate: 5/21/2024 Available Shifts: 8 D Pay Rate: $1542.00 - $1699.00 The Home Care RN Team Nurse is a registered nurse who applies nursing knowledge and clinical skills using the nursing process to provide high-quality, individualized, safe care to patients in their home environment The RN Team Nurse utilizes nursing assessment skills, critical thinking, problem-solving, and communication in providing home health services to patients. The Home Care RN Team Nurse works closely with the case manager, team members, and other health care providers to assess, plan implement and evaluate patient care with minimal direct supervision. Maintains availability at all times during work hours. Demonstrates continued re-assessment to determine the plan of care continues to meet the needs of the patient and communicates to all other health care providers including the home care team. Responsible to perform direct nursing care and documenting with accuracy. Acts as a patient advocate. Meets or exceeds productivity standards. Assists in developing and contributing to the patient plan of care with patients and/or caregivers that; Maintains the greatest amount of independence and enables the patient to remain safely in the home environment, follows care plans to achieve quality outcomes, assures attainable goals, provides continuity of care and coordination of care with other team members and health care providers, provides appropriate education to patients and caregivers that promotes health, wellness, and self-management. Regularly reassess to ensure services are appropriate, cost-effective, and not increasing the patient’s dependence. Assist patients/family in making informed decisions by ensuring the patient/family receives and understands appropriate information. Communicates effectively with leadership, team members, and other health care providers. Participates and communicates effectively during interdisciplinary team meetings. Initiates consultation with clinical managers as issues and concerns arise. Demonstrates leadership qualities including time management, verbal and written communication skills, listening skills, problem-solving, decision making, priority setting, work delegation, and work organization. Evaluates home health services provided by LPNs and home health aides according to agency policy, and documents and provide appropriate feedback promptly. Accurately completes and submits all required documentation according to policy. Clinical records accurately reflect patient assessment findings and interventions. Demonstrates proficiency with the use of multiple electronic devices and applications. Cares for and is responsible for company-issued electronic hardware/devices per policy. Responsible for proper use of supplies and equipment. Complies with agency budget. Supports and promotes a positive work environment. Demonstrates skill in conflict resolution. Supports communication of information regarding changes in agency operations and policies. Participates in the orientation of home care staff and provides input to the evaluation of orientation related to the specialty. Demonstrates knowledge of agency policies, procedures, and regulations regarding reimbursement for home care services and plans care accordingly. Demonstrates compliance with federal, state, and local rules and regulations, along with the organization's Corporate Responsibility Program. Attends and participates in staff meetings. Contributes and participates in continuous quality improvement activities. Participates and communicates in department/organizational committees.
