RTG Clinical Team will be conducting all phone screens. 

Dress Code:  Any color of professional scrubs.  No scrubs with other facility names or logos.  No sweatshirts or sweatpants allowed.  No restrictions on hair color, no offensive tattoos, and no color permitted on nails.

Department Notes

There are four hallways/neighborhoods.  Each hallway has their own dining area with a full kitchen.  There is a tub room and bath aide on staff who will take care of resident bathing.  Southridge - LTC/Rehab (13-15 residents); Morning View - Memory Care (9-10 residents); Sunshine - LTC (22 residents); Robinsview - LTC. 
ULC utilizes consistent staffing and prefers for agency to work in the same hallways when possible; however, agency will float to all hallways as needed. 

Ratios:  2-CNA per hallway during day and evening shifts; 1 CNA per hallway on nights; 1 RN per hallway on days and nights. 

Computer Charting:  Point Click Care.  There are kiosks on the wall within each hallway. 
Utilize Avera for Pharmacy services.  No IV medications administered; they use bubble packs for medication distribution

Communication:  RNs are provided with phones

Travel pairs are not permitted.

Specialty Profile Notes

Experience:  Minimum of 2 years experience working as a CNA in LTC/SNF setting. 
Registration to practice as CNA in State of South Dakota required. 

Shifts: Day 6:00am-2:30pm; Evening 2:15pm-10:45pm; Night 8:30pm-7:00am

Orientation:  Arrive 2 hours prior to shift for tour of building, overview of charting system, introduction to residents.
