Job Description

Responsible for direct patient care as well as telephone assistance with patients.

Update patient records.

Provide basic patient care as ordered, and

Administer medications and treatments as directed.

Travel to all five offices is a requirement; West Hartford, Avon, Manchester, Middletown, Hamden.

Administering medications and Immunotherapy injections

Documents assessments, interventions, patient/family responses, and medication dispensed/prescribed

Promotes wellness by providing patient education materials, communicating physician advice/instructions.

Adheres to infection control/safety guidelines and confidentiality policies

Qualifications and Job Info

1-2 years of recent experience

BLS certification

ACLS preferred

Active state license

MA/LPN/RN diploma/certification
