Job Summary
Join a dedicated healthcare team as a Licensed Practical Nurse (LPN) at a 120-bed Skilled Nursing Facility (SNF) located in Montour Falls, New York. This facility operates with three units, each accommodating up to 40 residents, fostering a close-knit and supportive environment.
In this role, you will provide essential person-centered care to a diverse patient population, assisting with assessments, care planning, implementation, and evaluation. You will be responsible for medication administration, treatment protocols, and meticulous documentation, collaborating closely with Registered Nurses and the broader interdisciplinary team to ensure the highest standards of patient well-being.
The evening shift offers a consistent schedule with a supportive staffing ratio of 2 LPNs and 3-4 CNAs per 40 residents, allowing for focused and effective patient care.
Key Responsibilities
•
Assist with the assessment, planning, implementation, and evaluation of comprehensive patient care plans.
•
Administer medications and treatments accurately and safely, adhering to facility policies and LPN scope of practice.
•
Monitor and record vital signs, observing and reporting changes in patient/resident conditions promptly to the Registered Nurse.
•
Collaborate effectively with the interdisciplinary care team, including RNs, CNAs, and other healthcare professionals, to coordinate patient care.
•
Document patient care activities, observations, and interventions thoroughly and accurately in the Electronic Medical Record (EMR).
•
Maintain clinical competence specific to the care area and complete all required organizational education within specified timeframes.
•
Practice ethically and professionally, advocating for patients and ensuring the provision of Person-Centered care and services.
•
Participate in interdisciplinary care team meetings and contribute to care planning discussions.
Required Qualifications
•
Current and active Licensed Practical Nurse (LPN) license.
•
Demonstrated experience working in a Skilled Nursing Facility (SNF) setting.
•
Ability to work a consistent evening shift schedule, including every other weekend and every other holiday.
•
Willingness and ability to float to other units as staffing needs dictate.
•
Ability to perform duties of a Certified Nursing Assistant (CNA) if staffing requirements necessitate.
Preferred Qualifications
•
Previous experience with Point Click Care EMR system.
