Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a Skilled Nursing Facility (SNF) in Montour Falls, New York. This facility comprises three units, accommodating a total of 120 residents, with each unit caring for up to 40 residents.
The LPN will play a crucial role in assisting with the assessment, planning, implementation, and evaluation of person-centered care for our geriatric patient population. Responsibilities include medication administration, vital sign monitoring, and comprehensive documentation within the Electronic Medical Record (EMR).
This position requires collaboration with a multidisciplinary care team, adherence to skilled nursing practice standards, and a commitment to patient advocacy. The successful candidate must be adaptable, willing to float to other units as needed, and prepared to assist in a Certified Nursing Assistant (CNA) capacity when staffing requires.
Key Responsibilities
•
Assist with the assessment, planning, implementation, and evaluation of patient care for all residents.
•
Administer medications and treatments under the supervision of a Registered Professional Nurse, in accordance with facility policy.
•
Monitor and record vital signs (respiration, temperature, blood pressure, heart rate) and promptly report changes in resident conditions.
•
Collaborate effectively with the interdisciplinary care team to ensure comprehensive and coordinated patient care.
•
Document patient care activities, observations, and changes in condition accurately and thoroughly in the Electronic Medical Record (EMR).
•
Maintain clinical competence specific to the care area and adhere to established standards of skilled nursing practice.
•
Practice ethically and professionally within the scope of practice, advocating for patients and providing Person-Centered care.
•
Keep the responsible Registered Nurse and other nursing personnel informed regarding changes in patient/resident conditions or other pertinent observations.
•
Float to other units as staffing permits and needs dictate.
•
Be prepared to function in a Certified Nursing Assistant (CNA) role as staffing permits and needs dictate.
Required Qualifications
•
Current LPN license in the State of New York.
•
Experience in a Skilled Nursing Facility (SNF) setting.
•
Ability to work every other weekend and every other holiday.
•
Ability to float to other units as needed.
•
Ability to perform duties in a CNA capacity if staffing requires.
Preferred Qualifications
•
Experience with Point Click Care EMR system.
Skills & Competencies

Shift Details
•
Hours: 3:00 PM - 11:30 PM (8.5-hour shifts)
•
Days per week: 5 days per week
•
Weekend Rotation: Every other weekend
•
Holiday Rotation: Every other holiday
•
Call Requirements: Not specified.
•
Contract Length: 13 weeks
•
Start Date: September 13, 2026
•
End Date: December 12, 2026
•
Orientation: 1 day on the assigned unit.
