Key Responsibilities of the RN - Case Management - Travel

Perform comprehensive assessment of high-risk members
•
Collaborate with primary care providers to implement individualized care plans
•
Address and reassess cost-efficient and appropriate levels of care
•
Coordinate non-medical support to ensure compliance with treatment plans
•
Coordinate care transitions and engage specialty resources as needed
•
Keep detailed records of clinical, functional, and fiscal outcomes
•
Act as patient advocate by protecting privacy and confidentiality issues
Requirements of the RN - Case Management - Travel
•
Associates or Bachelor's Degree in Nursing or related field
•
Current, unrestricted CO RN license
•
Ability to analyze complex medical information and make rational decisions
•
Case Management certification
