Supervise nursing assistants in resident care. Provide direct patient care, including bathing, dressing, grooming, and toileting assistance as needed. Monitor residents’ physical and emotional well-being, reporting any changes to the registered nurse or physician. Assist residents with mobility, transfers, and ambulation, ensuring safety and use of assistive devices. Perform vital signs, weight checks, and assist nursing assistants with activities of daily living. Administer medications as prescribed and document accurately. Provide wound care, including dressing changes, skin assessments, and preventive skin care. Collect specimens for lab testing. Monitor and report lab values and diagnostic results to the registered nurse or physician. Insert, maintain, and monitor urinary catheters, IVs, and feeding tubes as applicable. Administer oxygen therapy and monitor residents on oxygen. Document all care provided, observations, and resident changes in the medical record. Communicate effectively with the registered nurse, residents, and designated representatives. Report resident status changes to the supervising registered nurse or physician immediately. Participate in shift hand-off reports to ensure continuity of care. Supervise and direct nursing assistants in daily resident care tasks. Ensure nursing assistants follow care plans and report concerns. Perform skin observations during resident care and document any abnormalities. Provide feedback, support, and guidance to nursing assistants. Assist in reviewing and updating resident care plans under the supervision of a registered nurse. Participate in care conferences with team members. Implement interventions in accordance with care plans and physician orders. Recognize and respond promptly to medical emergencies with timely notification to the registered nurse. Assist in performing CPR and first aid as required. Contact the registered nurse, physician, or emergency services when necessary. Promote dignity, respect, and resident rights in all care activities. Provide emotional support to residents and their families. Educate residents and families about health needs, medications, and care routines within the practical nurse’s scope.
