Job Title: LPN Profession: Nursing Specialty: Long-Term Care (LTC) Duration: Part-Time Shift: Variable Day/Evening Hours per Shift: 8 hours Experience: 1 year as LPN License: NY LPN Licensure Certifications: BLS (AHA or RQI only) Must-Have: High School Diploma or GED Description: Under the direction of licensed nursing personnel, performs direct and indirect designated resident care within the scope of practice defined by certification. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit. Must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs. Provides the care needed by adult and geriatric patient groups. Resident's activities of daily living (ADLs) needs are met as outlined in the plan of care and in accordance with policies and procedures within a safe and comfortable environment. This includes hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, skin care, etc. Documentation is completed per protocol. Resident's environment and safety are maintained as outlined in the plan of care and in accordance with policies and procedures. This includes promptly answering call lights and alarms, provision of safety mats and low beds, restraint use under the direction of Nursing, maintaining cleanliness of the unit, behavior and fall prevention, and appropriate monitoring for malfunctioning equipment. Documentation is completed per protocol. Resident's nutritional needs are met as outlined in the plan of care and in accordance with policies and procedures. This includes meal delivery, tray preparation, adaptive equipment, feeding, and nourishments. Resident's preferences and choices shall be honored. Documentation is completed per protocol. Resident's rehabilitative (restorative and maintenance) needs are met as outlined in the plan of care and in accordance with policies and procedures. This includes range of motion, ambulation, transfers, proper body alignment, and use of assistive devices. Documentation is completed per protocol. Resident's rights, refusals, and concerns are reported to Charge and/or Supervising Nurse in a timely manner in accordance with policies and procedures. Changes in resident physical, emotional, and psychosocial status are identified and promptly reported to Charge Nurse. Occupational exposure to bloodborne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues. One year of experience in LTC or CNA capacity is required.
