Description: The Registered Nurse implements professional nursing practice centered on the patient as a whole person. The role focuses on the creation of a caring environment concentrated on healing and wellness. Nursing practices are based on evidence and current research, as well as hospital and department policies and procedures. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Responsibilities include coordinating, ensuring, and providing direct patient care activities. This includes provision of nursing rounds, assessments, treatments, and facilitating admission, transfer, and discharge activities. Patient and family-centered education and care coordination are integral to the role. The Registered Nurse conducts and documents comprehensive assessments in accordance with the RN scope of practice and hospital policies. Consideration of the patient’s culture, age, and developmental level is essential. The nurse identifies patient risk factors and presenting complaints. Health information is obtained from family or others as necessary. Objective and subjective data are collected, and initial admission assessments are verified. A problem list is developed, prioritizing patient needs based on inter-professional team input. The nurse designs interventions relevant to the patient’s culture and age. Desired outcomes are determined in coordination with the patient and family, following the medical plan of care. Full implementation of patient care therapies and procedures is expected, within the established policies and procedures. Explanations of procedures, treatments, and processes to patients and families are provided. The nurse initiates action to alleviate patient stress and discomfort. Recognition and appropriate communication of changes in patient condition are critical. Ongoing assessment and response to procedures and therapies are documented according to hospital policy. Tasks are delegated appropriately, with oversight of team members in patient care activities. The accuracy and completeness of delegated patient care are monitored. Educational needs of patients related to prioritized problems are identified and addressed. Facilitation of the patient discharge process is essential, ensuring timely execution of discharge orders and teaching. Input into patient/nurse assignments is provided, considering available staffing and patient acuity. Participation in unit-based activities in support of organizational values/priorities is expected. A positive group dynamic and healing environment are promoted through actions and words. Nursing excellence is assured in accordance with accepted standards and evidence-based practice. Active participation in quality improvement activities and core measures is required. A holistic perspective regarding patient care is utilized.
