Assessment of patient needs involves comprehensive evaluations in accordance with the RN's scope of practice. Consideration of the patient's culture, age, and developmental level is crucial. Patient risk factors and presenting complaints must be identified. Patient health information is obtained from family or others as necessary. Both objective and subjective data are collected. Initial admission assessments must be verified. A prioritized problem list is developed based on input from the inter-professional team as appropriate. Actions and interventions are designed relevant to the patient’s culture and age. Desired outcomes are determined in coordination with the patient and family in accordance with the medical plan of care. Patient care therapies and procedures are fully implemented with empowerment and clinical autonomy. Procedures, treatments, and processes are explained to the patient and family. Initiating actions to alleviate patient stress and discomfort is part of the role. Changes in patient conditions must be recognized, responded to, and communicated appropriately. Ongoing assessment and response to procedures and therapies is documented using the nursing process. Tasks are delegated appropriately, and oversight of team members is provided. The accuracy and completeness of delegated patient care must be monitored. Educational needs of patients related to prioritized problems are identified and addressed. The patient discharge process is facilitated by executing discharge orders in a timely manner. Input into patient/nurse assignments is given while considering available staffing, unit activity, and patient acuity. Active participation in unit-based activities in support of organizational values and priorities is expected. A positive group dynamic and healing environment are promoted through words and actions.
