Description: The RN implements professional nursing practice centered on the patient as a whole person. A caring environment focused on healing and wellness is created. Nursing practices are based on evidence and current research as well as hospital and department policies and procedures. Regulatory standards and professional guidelines are followed. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Coordinates, ensures, and provides direct patient care activities on the unit. This includes provision of nursing rounds, assessments, and treatments. Facilitates admission, transfer, and discharge activities. Provides patient and family-centered education. Engages in care coordination and individualized treatment planning as applicable. Assessment of patient needs is performed and documented in accordance with RN's scope of practice and hospital policies and procedures. Cultural, age, and developmental considerations are made for each patient. Patient risk factors and presenting complaints are identified. Patient health information is obtained from family or others as necessary. Objective data is collected along with subjective data. Initial admission assessments are verified if obtained by other members of the healthcare team. A problem list is developed, prioritizing patient needs based on input from the inter-professional team as appropriate. Actions and interventions relevant to patient culture and age are designed. Desired outcomes are determined in coordination with the patient and family per the medical plan of care. Patient care therapies and procedures are fully implemented. Empowerment and clinical autonomy within scope of practice are exercised according to established policies, procedures, and professional guidelines. Procedures, treatments, and processes are explained to the patient and family. Action is initiated to alleviate patient stress and discomfort. Changes in patient condition are recognized, responded to, and communicated appropriately. Ongoing assessment and response to procedures and therapies are conducted and documented. Task delegation takes place appropriately, overseeing team members’ performance concerning patient care. Accuracy and completeness of delegated patient care are monitored. Educational needs of patients related to prioritized problems are identified and addressed. The patient discharge process is facilitated by executing discharge orders in a timely manner. Potential discharges are identified, and discharge teaching is performed while coordinating the plan with case management as indicated. Input into patient/nurse assignments is provided, considering available staffing, unit activity, and patient acuity. Active participation in unit-based activities in support of organizational values and priorities is demonstrated. A positive group dynamic and healing environment are promoted through words and actions. Nursing excellence is assured in accordance with accepted standards, evidence-based practice, and current research. Participation in department and unit quality improvement activities and core measures is actively engaged in, including patient satisfaction, patient falls, pressure ulcers, and auditing and monitoring of documentation. A holistic perspective regarding patient care is utilized.
