The Registered Professional Nurse assesses patient needs, develops a plan of care in collaboration with the interdisciplinary team, provides direct care, evaluates patient progress, and coordinates care delivery throughout the patient’s episode. The RN develops specialized skills in the area of patients admitted for medical conditions. The nurse provides care to adult, adolescent, and geriatric populations with medical conditions. The RN is accountable for patient and family education to assist in understanding self-care beyond hospitalization, including lifestyle changes to support health and healing. The Registered Nurse is expected to promote comfort and provide emotional support in a healing environment. Job Functions: Applies nursing knowledge and skill in the delivery of care. Assesses patient and family needs through active listening and analyzing data obtained through direct observation, assessment, and communication. Creates a developmentally, age-appropriate, culturally relevant, individualized plan of care to address health problems identified. In collaboration with the patient, family, physician, and healthcare team members, sets goals and monitors patient progress. Adjusts the plan based on patient response or change in condition and reprioritization of needs. Promotes and advocates for the health, safety, and rights of the patient. Encourages mutual trust, respect, and cooperation among team members to achieve the best possible patient outcomes. Teaches others as appropriate and delegates tasks consistent with training and scope of practice. Understands the responsibility to safeguard public safety through compliance with infection prevention strategies. Motivates the patient and family by teaching them what is needed to continue care and healing at home. Assesses the patient and family learning styles and structures sessions to maximize understanding. Seeks teachable moments to emphasize pertinent information and provides tools and information to assist the family after discharge. Documents education provided and patient/family response. Evaluates quality and effectiveness of nursing practice, using results of quality improvement activities to initiate changes. Engages in self-evaluation of practice, identifying strengths and areas for professional development. Takes accountability to increase knowledge and enhance skills. Uses logic and reasoning to identify strengths and weaknesses of alternative solutions to problems. Nursing practice reflects the importance of making meaningful connections with patients and families.
