Description: The Registered Nurse implements professional nursing practice centered on the patient as a whole person and the creation of a caring environment focused on healing and wellness. Nursing practices are based on evidence and current research as well as hospital and department policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Coordinates, ensures, and provides direct patient care activities through assessment and treatment. Facilitates admission, transfer, and discharge activities with a focus on patient and family-centered education. Identifies patient risk factors, collects data, and develops a problem list prioritizing patient needs. Designs actions and interventions relevant to the patient’s age and culture. Determines desired outcomes in coordination with the patient and family. Implements patient care therapies and procedures within scope of practice according to established policies and guidelines. Communicates procedures and treatments to patients and families and addresses associated stress and discomfort. Recognizes changes in patient conditions and communicates effectively. Documents ongoing assessments and responses to procedures and therapies. Delegates tasks appropriately and oversees team members in patient care activities. Monitors the accuracy and completeness of delegated patient care provided by other team members. Identifies educational needs of patients related to prioritized problems based on comprehension levels. Facilitates patient discharge processes and ensures timely execution of discharge orders. Provides input into patient/nurse assignments considering staffing, unit activity, and patient acuity. Participates actively in unit-based activities in support of organizational values and priorities. Promotes a positive group dynamic and healing environment through words and actions. Assures nursing excellence in accordance with accepted standards and evidence-based practices. Actively participates in quality improvement activities and core measures related to patient outcomes.
