Practicing within scope, standards, policies, values, and mission of the organization. Plans, implements, and evaluates patient care plans based on patient assessment to optimize outcomes and maximize available resources. Requires prioritization and critical thinking skills, decisive judgment, and ability to work with minimal supervision in a fast-paced environment. Complies with triage, treatment, evaluation, and documentation guidelines. Uses evidence-based practices and competence to collaborate with the healthcare team to provide quality outcomes and coordinate patient care. Coordinates patient transfer, discharge, referral, and spiritual/psychosocial support needs. Identifies barriers to effective communication based on psychosocial, literacy, financial, and cultural considerations. Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstance. Effectively delegates patient care to ancillary personnel. May precept student nurses and new hires. Practices according to professional and ethical standards as noted in the Code of Ethics. Demonstrates caring attitudes and behaviors to develop therapeutic relationships. Engages in patient and family teaching as appropriate. Demonstrates full commitment to organization values.
