Description: The RN implements professional nursing practice centered on the patient as a whole person. This role focuses on the creation of a caring environment centered on healing and wellness. Nursing practices are based on evidence and current research, as well as hospital and department policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Coordinating, ensuring, and providing direct patient care activities includes: provision of nursing rounds, assessments, and treatments. Responsibilities include facilitating admission, transfer, and discharge activities. The RN performs and documents a comprehensive assessment in accordance with scope of practice and hospital policies/procedures. They consider the patient’s culture, age, and developmental level. Identifying patient risk factors and presenting complaints is essential. Collecting objective and subjective data and verifying initial assessments obtained by other healthcare team members are key tasks. Developing a prioritized problem list based on input from the inter-professional team is necessary. Actions and interventions are designed relevant to the patient’s culture and age. Determining desired outcomes in coordination with the patient and family according to the medical plan of care is also required. The RN fully implements patient care therapies and procedures with empowerment and clinical autonomy within scope of practice in accordance with established policies and professional guidelines. They explain procedures, treatments, and processes to patients and families. Initiating actions to alleviate patient stress and discomfort is crucial. Recognizing, responding to, and communicating changes in patient condition appropriately is expected. Conducting and documenting ongoing assessments and responses to procedures and therapies according to hospital policy is required. Delegating tasks appropriately and overseeing team members in the performance of patient care activities is part of the role. Monitoring the accuracy and completeness of delegated patient care provided by other team members is essential in implementing the plan of care. Identifying and addressing educational needs of patients related to prioritized problems according to their level of comprehension is necessary. Facilitating the patient discharge process by executing discharge orders in a timely manner is expected. Providing input into patient/nurse assignments based on available staffing, unit activity, and patient acuity is part of the job.
