Key Responsibilities of the RN - Case Management - Travel

Collaborate with the healthcare team to manage patient readmission initiatives in the ED
•
Assess, plan, and coordinate patient care to ensure seamless transitions
•
Engage in floating duties, potentially covering other campuses as required
•
Function effectively within a multidisciplinary team to deliver high-quality patient care
Requirements of the RN - Case Management - Travel
•
FL/Compact nursing license (recent Nursys)
•
2 years of Case Manager experience, acute care experience required
•
Prior travel experience required
•
Degree accreditation on profile or 5 years of experience
