RN CASE MANAGER
UNI Healthcare is looking for compassionate and dedicated RN Case Manager to join our hospice care team. As an RN in our hospice program, you will play a crucial role in providing comprehensive and compassionate care to terminally ill patients and their families.
Job Description:
As a Hospice Case Manager, you will play a crucial role in providing holistic care and support to our hospice patients and their families. You will be responsible for assessing patient needs, developing care plans, coordinating services, and advocating for the best possible outcomes for our patients. The ideal candidate will have a strong background in nursing or social work, excellent communication skills, and a compassionate approach to end-of-life care.
Responsibilities:
- Conduct initial assessments of patients to determine their physical, emotional, and spiritual needs
- Develop comprehensive care plans in collaboration with the interdisciplinary team
- Coordinate and monitor services provided to patients, including nursing care, medical treatments, and psychosocial support
- Provide education and support to patients and families on end-of-life care options
- Advocate for patients' rights and preferences in care decisions
- Collaborate with healthcare providers, community resources, and other stakeholders to ensure continuity of care
- Maintain accurate and timely documentation of patient assessments, care plans, and interventions
Qualifications:
