Specialty: RN Case Management

Job Details:

• Provides nursing case management and delegating RN services to a caseload of individual’s with autism and related disabilities, in our school and residential program.
• The Delegating Registered Nurse provides services that include:
  • assessing individuals’ health needs
  • developing care plans
  • visiting homes and supervising direct support professionals in implementing delegated nursing tasks and medication administration.
• Serves as liaison with physicians, other healthcare professionals, and family members.
• Maintains appropriated nursing documentation including 45 day reviews.
• Provides staff training.
• Participates as an integral member of each individual’s support team.
• Must be available for on-call responsibilities, with flexibility with day/evening hours

Requirements:

• Two (2) years’ experience working in human services and with individuals with developmental disabilities
• Case Management or Utilization Review Certification - Highly Preferred
• DDA-Delegating Nurse experience preferred or must be willing to complete the MBON mandated DDA RN Case Manager/Delegating Nurse/MTTP Trainer Course within 3 months of hire
• Valid driver’s license with clean driving record-local travel required
