Job Overview

We are seeking a Licensed Practical Nurse (LPN) for a local contract assignment in a skilled nursing and long-term care setting. The LPN will provide direct patient care, administer medications, monitor resident conditions, and support the nursing team in delivering safe and effective care.

This role is suited for LPNs who are comfortable working structured day shifts in a long-term care environment.

Key Responsibilities

Administer medications and prescribed treatments
Monitor, assess, and document patient conditions and changes
Provide direct nursing care according to care plans
Assist with admissions, discharges, and resident evaluations
Communicate patient updates to supervising RN and care team
Maintain accurate and timely clinical documentation
Support infection control and safety protocols
Collaborate with interdisciplinary healthcare staff

Requirements

Active Licensed Practical Nurse (LPN) license required
Long-term care experience preferred
Strong clinical and communication skills
Ability to work every other weekend as required
Must comply with onboarding requirements
Must bring state or federally issued identification and wear a photo badge at all times
COVID-19 vaccination or approved exemption may be required per policy
