Job Overview

The LPN will provide direct patient care, administer medications, monitor resident conditions, and support the interdisciplinary care team in delivering exceptional resident-centered care.

Key Responsibilities

Administer medications and treatments as prescribed
Monitor and document resident conditions and changes
Provide direct nursing care according to individualized care plans
Assist with admissions, discharges, and care coordination
Communicate patient updates to supervising nursing staff and physicians
Maintain accurate and timely clinical documentation
Follow infection prevention and resident safety protocols
Collaborate with interdisciplinary healthcare teams to support quality care

Requirements

Active Licensed Practical Nurse (LPN) license required
Previous long-term care or skilled nursing experience preferred
Strong clinical assessment and communication skills
Ability to work effectively within a healthcare team
Must comply with onboarding requirements
Must bring state or federally issued identification and wear a photo badge at all times
COVID-19 vaccination or approved exemption may be required per policy
