Job Overview

We are seeking a Licensed Practical Nurse (LPN) for a local contract assignment in a skilled nursing and rehabilitation setting. The LPN will provide direct patient care, administer medications, monitor patient conditions, and support the nursing team in delivering safe and effective care.

Key Responsibilities

Administer medications and treatments as prescribed
Monitor and document patient conditions and changes
Provide direct nursing care according to care plans
Assist with admissions, discharges, and care coordination
Communicate updates to supervising RN
Maintain accurate clinical documentation
Support infection control and safety protocols
Collaborate with interdisciplinary healthcare teams

Shift: Night Shift
Shift Time: 22:00 06:30

Guaranteed Hours: 40 Hours per Week

Requirements

Active Licensed Practical Nurse (LPN) license required
Long-term care experience preferred
Strong clinical and communication skills
Every weekend commitment required
All requested time off (RTO) must be approved prior to contract finalization
Must have reliable personal transportation (public transportation in the area is limited and unreliable)
No overtime available
Must comply with onboarding requirements
Must bring state or federally issued identification and wear a photo badge at all times
COVID-19 vaccination or approved exemption may be required per policy
