KEY RESPONSIBILITIES

Provide comprehensive hospice nursing care to terminally ill patients in home and facility settings
Conduct initial and ongoing assessments; manage pain and symptom control
Educate patients and family caregivers on disease progression and end-of-life care
Coordinate with hospice interdisciplinary team including social work, chaplaincy, and aide services
Document clinical assessments, medications, and visit notes in EMR
