• Registry personnel must bring a state or federal issued photo ID to the facility and wear a photo badge at all times
• Agency staff must have completed the original or updated COVID-19 vaccine series, or present a documented religious and/or medical exemption from their employer, to be considered for any assignment
• Additional vaccine doses must be obtained where required by law
• Every weekend commitment required
• All requests for time off (RTO) must be approved before contract is finalised
• Overtime is not available for this assignment
• Candidate must have own reliable transportation public transportation is limited and unreliable in this area

Requirements

Must-Have

• Active Vermont LPN/LVN licence current and in good standing
• CPR/BLS certification current
• Minimum 1 year of LPN experience in a Skilled Nursing / Long-Term Care setting
• Reliable personal transportation public transportation in this area is limited and unreliable
