We are seeking a dependable Licensed Practical Nurse (LPN) for a local contract assignment in a skilled nursing facility. The LPN will provide direct patient care, administer medications, and support the interdisciplinary care team to ensure high-quality resident outcomes.

• Provide direct nursing care under RN supervision
• Administer medications and treatments as prescribed
• Monitor and document patient conditions and changes
• Assist with admissions, discharges, and care planning
• Support resident daily care needs as required
• Maintain accurate and timely clinical documentation
• Ensure compliance with facility and regulatory standards

• Active Licensed Practical Nurse (LPN) license
• Current BLS certification (if required)
• Strong clinical and communication skills
• Experience in long-term care preferred
• Ability to work in a structured healthcare environment
• Reliable transportation for local contract assignment
