Job Overview

We are seeking a Licensed Practical Nurse (LPN) for a local contract assignment in a skilled nursing facility. The LPN will provide direct patient care, administer medications, and support residents daily health needs while working within a multidisciplinary care team.

Key Responsibilities

Administer medications and treatments as ordered
Monitor patient conditions and report changes
Provide direct nursing care and ADL support
Maintain accurate and timely clinical documentation
Support infection prevention and safety protocols
Collaborate with RNs and healthcare team members
Ensure resident comfort, dignity, and safety

Requirements

Active LPN/LVN license (state appropriate)
Long-term care experience preferred
Strong communication and clinical skills
