Job Overview

We are seeking a Licensed Practical Nurse (LPN) to provide compassionate, high-quality nursing care within a skilled nursing facility. This role involves delivering direct patient care, administering medications, monitoring patient conditions, and supporting the interdisciplinary healthcare team to ensure exceptional resident outcomes.

The LPN will play an essential role in maintaining a safe, patient-centered environment while promoting quality care and clinical excellence.

Key Responsibilities

Provide direct nursing care in accordance with individualized care plans
Administer medications and prescribed treatments safely and accurately
Monitor patient conditions and report changes to the healthcare team
Document patient care and clinical observations in EMR systems
Assist with admissions, discharges, and ongoing patient assessments
Support infection prevention and patient safety initiatives
Collaborate with physicians, RNs, CNAs, and other healthcare professionals
Promote patient comfort, dignity, and quality of care

Requirements

Active Licensed Practical Nurse (LPN) license in New Mexico
State-issued identification required
