Job Description

We are seeking a compassionate and organized PACE Home Care Coordinator (CNA/HHA) to support the coordination of home-based services for older adults. Working as part of an interdisciplinary care team, you will help ensure participants receive high-quality, person-centered care by coordinating home care services, conducting home safety evaluations, and serving as a liaison between participants, caregivers, families, and service providers.

This role is ideal for a CNA/Home Health Aide who is passionate about caring for the elderly and enjoys working both in the community and in a collaborative healthcare environment.

Responsibilities

  • Coordinate home care services, meal deliveries, caregiver services, and durable medical equipment (DME).
  • Conduct home safety assessments during enrollment, annual, semi-annual, and change-of-address visits.
  • Complete and maintain participant care plans in collaboration with the interdisciplinary team.
  • Perform monthly home visits for homebound participants and document findings accurately.
  • Monitor service delivery, identify gaps in care, and communicate updates to the care team.
  • Maintain accurate documentation in the electronic medical record (EMR).
  • Coordinate with caregivers, vendors, and healthcare providers to ensure timely and effective service delivery.
  • Advocate for participants' healthcare needs and preferences while promoting person-centered care.

Requirements

  • Current Certified Nursing Assistant (CNA) certification.
  • Current California Department of Public Health Home Health Aide (HHA) certificate.
  • Current American Heart Association (AHA) CPR and First Aid certification.
  • Minimum 1 year of experience working with the frail or elderly population.
  • Valid California Driver's License with reliable transportation.
