Key Responsibilities of the RN - Home Health - Travel

• Provide high-quality home health and hospice care
• Manage a caseload of approximately 5-6 patients per day
• Conduct patient assessments and develop care plans
• Coordinate with unit secretaries and techs as required
• Document patient care using the HCHB system

Requirements of the RN - Home Health - Travel

• Valid RN license
• Home health and hospice experience
• Prior experience in the Franklin/Macon county area preferred
• Reliable vehicle for patient visits
