Job Title: LPN Home Health Profession: LPN Home Health Specialty: LPN Home Health Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: Home care experience required License: Current LPN license Certifications: BLS Must-Have: - Home care experience - Ability to perform skilled nursing functions - Team-oriented mindset Description: The LPN position provides six patient visits per day. The role involves working within teams of RNs, PTs, OTs, and HHAs. This position requires experience in home care. Under the direction of the Clinical Team Manager, responsibilities include performing skilled nursing functions in accordance with the State Nurse Practice Act. The LPN will ensure compliance with federal, state, and local laws, as well as organizational and departmental policies and procedures. The LPN delivers care to patients as delegated by the RN Case Manager and/or Clinical Team Manager. This position requires working one weekend shift per month.
