Location: Grand Junction, CO

Pay: $55 to $60 per hour
Shift Information: Days - 5 days x 8 hours

About the Position

Position Summary:
The Primary RN Case Manager is responsible for planning, coordinating, and providing skilled nursing care to hospice patients in community settings. This role serves as the primary clinical contact for assigned patients, ensuring individualized, compassionate, and high-quality end-of-life care while supporting families and caregivers.

Key Responsibilities

• Conduct comprehensive initial and ongoing patient assessments.
• Develop, implement, and update individualized hospice care plans.
• Manage a caseload of hospice patients receiving care in the community.
• Coordinate care with physicians, nurse practitioners, social workers, chaplains, home health aides, therapists, and volunteers.
• Provide symptom management, pain control, and comfort-focused nursing care.
• Educate patients, families, and caregivers about disease progression, medications, symptom management, and end-of-life care.
• Administer medications and treatments according to physician orders and hospice protocols.

Requirements

Qualifications

Required

• Current Registered Nurse (RN) license.
• Minimum of 1–2 years of clinical nursing experience.
• Valid driver's license and reliable transportation (for community visits).
• Current Basic Life Support (BLS) certification.
• Strong assessment, communication, and critical thinking skills.
