• Active LPN license in Tewksbury.
• 1+ year experience in LTC/SNF preferred
• Current BLS certification
• Familiarity with EMRs (e.g., PointClickCare) preferred
• Strong communication and teamwork skills

• Deliver routine nursing care and support daily health needs
• Administer medications and treatments as prescribed
• Monitor and document changes in resident condition
• Assist with ADLs and mobility as needed
• Maintain accurate records in EMR systems
• Communicate with residents, families, and interdisciplinary teams
• Support infection control and safety protocols
• Report concerns or changes to RN/supervisor promptly
• Participate in shift handoffs and team meetings
