We are seeking an experienced and compassionate Licensed Practical Nurse (LPN) Team Leader to support a respected nursing home in Hollidaysburg, Pennsylvania. Position Details

• Position: LPN Team Leader
• Location: Hollidaysburg, PA
• Schedule: Per Diem
• Shift Needed:  Multiple Shifts
• Hours: 8–16 hours per week
• Pay: $35.00 - $37.00 per hour

Responsibilities

• Supervise and support nursing assistants and care staff during assigned shifts
• Administer medications and treatments as prescribed
• Monitor resident conditions and report changes appropriately
• Coordinate resident care with interdisciplinary team members
• Ensure compliance with facility policies, procedures, and state regulations
• Complete accurate documentation and charting
• Promote a positive, resident-centered environment
• Assist with admissions, discharges, and care planning as needed

Requirements

• Current Pennsylvania LPN License in good standing
• Minimum of 2 years of nursing experience in Long-Term Care, Skilled Nursing, or Rehabilitation
• Strong leadership and communication skills
• Ability to work independently and as part of a team
• Excellent clinical and organizational skills
