Seven Healthcare are seeking an experienced Registered Nurse with a specialty in case management to join our team in Waterford, WI. This is a travel contract position, primarily focusing on providing visit-based care. The role involves working within a community setting to ensure comprehensive assessments and effective patient care planning.

Key Responsibilities of the RN - Case Management - Travel

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers to implement individualized care plans
• Address and reassess cost-efficient, appropriate levels of care
• Coordinate care transitions as needed
• Engage specialty resources to achieve optimal resolution
• Keep detailed records of clinical, functional, and fiscal outcomes
• Identify opportunities for health promotion and illness prevention
• Act as patient advocate by protecting privacy and confidentiality issues

Requirements of the RN - Case Management - Travel

• RN License for WI
• Hospice and wound care experience
• Experience with Home Care Home Base (HCHB) highly desired
• Certified CPR/BLS
• Valid driver's license and reliable vehicle with valid car insurance
• Completed skills competency checklist
• Training in elder abuse, HIPAA, infection prevention, and OSHA
• Case Management certification
• Associates or Bachelor's Degree in Nursing
