Job Overview

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality nursing care in a skilled nursing and long-term care setting. The Licensed Practical Nurse will play an essential role in monitoring patient conditions, supporting care plans, and maintaining a safe and patient-centered environment.

Key Responsibilities

Provide direct nursing care to patients under the supervision of a Registered Nurse or physician
Administer medications and treatments as prescribed
Monitor, assess, and document patient conditions and responses to treatment
Assist with developing and implementing individualized care plans
Maintain accurate and timely clinical documentation
Educate patients and families regarding treatments and ongoing care
Collaborate with interdisciplinary healthcare team members
Ensure compliance with infection prevention and safety protocols

Requirements

Graduate of an accredited Practical Nursing program
Active Licensed Practical Nurse (LPN) license in the state of Pennsylvania
