About the Position

🚨 Now Hiring: PACE Admissions Registered Nurse (RN) – San Diego, CA 🚨

💰 Pay: $60–$68/hour
📍 Location: San Diego, CA
📅 Contract: 3 Months (High Possibility of Extension)
🕒 Schedule: Monday–Friday | 8:00 AM – 5:00 PM
⏱️ Guaranteed: 40 Hours/Week
🚫 No Weekends | No On-Call

We're looking for an experienced PACE Admissions RN to join a dedicated healthcare team serving the geriatric community. This role involves conducting in-home patient assessments, completing Level of Care (LOC) evaluations, and coordinating care for elderly patients.

Requirements:

✅ Active California RN License
✅ Minimum 1 year of RN Home Health experience
✅ Experience with comprehensive nursing assessments
✅ Ability to complete Level of Care (LOC) evaluations
✅ Valid driver's license and reliable transportation
✅ Strong documentation and care coordination skills

Preferred:

✔️ Home Health or Community Health experience
✔️ Case Management experience
✔️ Experience working with the frail elderly population
✔️ DME and home care coordination experience
