About the Position
Job Title: Home Health RN Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Tuesday - Saturday Hours per Shift: 8am - 4pm Experience: Minimum 2 years License: Valid RN License Certifications: CPR Certification Must-Have: - Experience with OASIS documentation - Knowledge of evidence-based nursing practices - High-Tech nursing skills, including IV use and complex wound care Description: Hospice coverage required when on-call. Effectively completes documentation in accordance with agency policies and regulations. Demonstrates knowledge of and stays up-to-date on OASIS documentation requirements. Utilizes the nursing process to assess patient and caregiver needs. Develops and communicates a realistic plan of care for each patient. Serves as case manager for assigned patients, managing caseloads in accordance with productivity standards. Implements medical plans of care and necessary nursing interventions to ensure quality of care. Stays current on high-tech skills such as IV use, maintenance, wound VAC, and complex wound care. Maintains infection control standards throughout the plan of care. Works collaboratively with LPNs to ensure the care plan is followed and that patient needs are met. Evaluates various aspects of the implemented plan of care and patient responses. Maintains accurate documentation to ensure integration of information for ongoing quality of care. Follows agency protocol with necessary processes for documentation and visit completion. Performs patient and caregiver education to ensure informed decision-making. Identifies inter-disciplinary needs of the patient and communicates them in a timely manner. Accurately completes a variety of forms in a timely manner. Carries out visits efficiently, ensuring supervisory visits are appropriately scheduled. Assesses patients' needs for services, developing and coordinating plans of care. Transmits patient incident reports as necessary in a timely manner. Coordinates safe patient discharges or transfers and ensures processing of paperwork. Coordinates patient care with physicians, providers, and team members to support the plan of care. Participates in peer review audits and accepts feedback constructively. Schedules and facilitates case conferences with team members to maximize communication. Advocates for patient care issues to ensure sensitivity to specific patient and family needs.
