About the Position

Job Title: Outpatient RN Care Manager Profession: Registered Nurse Specialty: Outpatient Clinic Duration: 12 weeks Shift: Day 5x8-Hour Hours per Shift: 8 hours Experience: Minimum of 1 year of prior RN experience required (clinic, primary care, pre/post, home health settings preferred) License: CA license required at time of start Certifications: BLS required Must-Have: EPIC experience required Description: The Registered Nurse Care Manager will case manage participants and their medical needs in the clinic and in the home for primary care needs. Support the provider, do initial and reassessments as per regulations. Assist with level of care determination assessments required to determine eligibility for the program. Conduct some lab procedures. Assist with medication management. Participate in care planning and be part of the interdisciplinary team. Daily patient ratio is 1:40. Oasis experience is required. Candidate is expected to have reliable transportation and no additional scheduling restrictions. All travelers are expected to float between all facilities within a given state. Travelers' refusal to float may result in termination of assignment. Providers will be responsible for submitting monthly cell phone bills directly to the agency.
