Responsibilities:
- Conduct comprehensive assessments of residents' physical, mental, and emotional health to develop and implement individualized care plans, ensuring optimal patient outcomes.
- Provide prescribed medications and treatments, monitor for side effects or reactions, and ensure compliance with treatment plans for chronic conditions, rehabilitation, or post-acute care.
- Continuously monitor residents' vital signs, wounds, and overall conditions, providing appropriate interventions and support, and reporting any changes to the healthcare team.
- Work with physicians, therapists, and other healthcare professionals to coordinate care and provide education and emotional support to families on patient progress and care requirements.
- WI Registered Nurse license
- Basic Life Support (BLS) certification
- 1-2 years of experience working in a skilled nursing facility or with geriatric populations
- Pay: $1,961.57 - $2,022.24 / week
- Location: Hayward, WI
- Assignment Length: 13
- Shift: Flex - up to 36.00 hours per week
