Candidates must possess a current RN licensure in Client. Candidates should have two years of experience in a home health setting within the last fifteen years. Recent experience conducting comprehensive assessments of patients is required. Ability to review health status questions and make evaluations and determinations is essential. Candidates should be proficient in using technology in the workplace. Knowledge of nursing theory, principles, and practices is necessary. Candidates must understand the laws and regulations governing professional nursing practices. Excellent organizational, interpersonal, written, and verbal communication skills are required. Candidates must be able to communicate effectively with diverse consumers. Sensitivity to the concerns of others is important. Candidates should perform comfortably in fast-paced, deadline-oriented environments. Successful execution of complex tasks simultaneously is essential. Independent work is a key responsibility. Responsibilities include performing initial in-home pre-evaluations of patients. Evaluations will assess health status, strengths, care needs, and preferences. These evaluations will guide the development of individualized long-term care service plans. Candidates will review consumer medical documentation as relevant to cases. Data from evaluations must be entered into an electronic format and transmitted as required. Documenting concerns and conflicting information during evaluations is necessary. Emphasizing continuity of care is critical to reducing gaps in treatment plans. Candidates will discuss healthcare options, supports needed, and service vendor options with consumers. Initiating communication with eligible consumers for enrollment into managed care health plans is a responsibility. Enhancing communication and collaboration with interdisciplinary care team members is essential to improve care coordination. Collecting quality review data and required documentation for outcome measurements is necessary. Candidates will record case notes into the customer relationship management system. Identifying opportunities for health promotion and illness prevention is part of the role. Maintaining a comprehensive knowledge of community resources and network services for the target population is expected. Candidates must meet performance standards for this position. Other duties may be assigned by the Regional Nurse Manager or other project management. All candidates must conduct in-field assessments as part of their responsibilities.
