About the Position

Job Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: One year of acute nursing care required, experience in home health preferred. License: Massachusetts LPN license required. Certifications: Active American Heart Association BLS or American Red Cross Certification required. Must-Have: - Ability to function independently under general guidance. - Ability to maintain confidentiality. - Critical thinking skills. - Emotional stability in emergency settings. - Valid driver's license and insurance. - Own vehicle for transportation. Description: The Community Health LPN provides and documents nursing care for the sick and injured in a home setting. The LPN intervenes to promote, maintain, or restore health, to prevent illness, and to support rehabilitation. The LPN collaborates with healthcare providers and community representatives to coordinate home care. The LPN evaluates individual and family responses to interventions. The role requires independent action and judgment in providing care. Time management, communication, planning, delegating, and problem-solving skills are essential. The candidate must demonstrate critical thinking abilities and maintain emotional stability in urgent situations. Flexibility regarding work hours is required, including willingness to accept on-call responsibilities. The candidate must have demonstrated interpersonal skills and a history of dependability in meeting work schedules. The role involves documenting clinical data both verbally and in written form. Requests for time off must be noted prior to submission. No more than five days of requested time off will be accepted. Demonstrated ability to perform independently as a clinician with minimal supervision is necessary. Knowledge of home care reimbursement and regulatory guidelines is essential for effective service delivery. Experience with self-scheduling of caseload is preferred.
