Job Title: LPN Home Health Profession: LPN Home Health Specialty: LPN Home Health Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: Home care experience required License: Valid LPN license Certifications: BLS certification required Must-Have: Ability to work within a team and provide skilled nursing care Description: The LPN position requires providing care for six patient visits per day. The LPN works within teams that include RNs, PTs, OTs, and HHAs. Under the direction of the Clinical Team Manager, the LPN is responsible for performing skilled nursing functions. This is done in accordance with the State Nurse Practice Act and established standards of practice. Compliance with federal, state, and local laws is essential. The LPN delivers care to patients as delegated by the RN Case Manager and/or Clinical Team Manager. This position requires working one weekend shift per month. Candidates must be able to follow established organizational and departmental policies and procedures.
