Job Overview

Modoc Medical Center is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join its Skilled Nursing Facility team. The LVN will work under the direction of the Director of Nursing (DON) and Registered Nurses to provide exceptional, patient-centered care while collaborating with physicians, nursing staff, residents, and families.

This is an excellent opportunity for an LVN looking for a rewarding travel assignment in a supportive healthcare environment.

Responsibilities

• Deliver high-quality nursing care to SNF residents.
• Perform patient assessments and report changes in condition.
• Administer medications safely according to physician orders.
• Develop and implement individualized care plans.
• Monitor and document patient progress accurately.
• Assist with treatments, wound care, IV therapy, blood draws, EKGs, and other nursing procedures within scope of practice.
• Respond effectively to emergency situations.
• Educate patients and families regarding treatment plans.
• Maintain infection prevention and patient safety standards.
• Collaborate with interdisciplinary healthcare teams.
• Support CNAs and other nursing staff to ensure quality resident care.
• Maintain accurate medical documentation.
• Participate in staff meetings, continuing education, and orientation of new employees.

Requirements

• Active California LVN License
• Minimum 1 year of LVN experience
• Current BLS (CPR) Certification
• Strong assessment and communication skills
• Ability to work independently and as part of a team
• Excellent documentation and time-management skills
• Must be dependable with a strong attendance record
