Job Title: Clinical Nurse Profession: RN Specialty: Rehab Duration: 9 months Shift: Day Shift Hours per Shift: 12.5 hours Experience: Minimum 2 years RN experience, 2 years rehabilitation and medical surgical experience, 1 year travel experience preferred for travel positions License: NYS RN License Certifications: BLS, CRRN preferred Must-Have: Rehabilitation Nursing Medical-Surgical Nursing Patient Care Management Critical Thinking Patient Assessment Description: The Clinical Nurse is a Registered Professional Nurse who provides safe and competent quality care based on nursing theory and research to a designated group of patients and their significant others. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care. Utilizes the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. Assesses and evaluates patient care needs while applying critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after-hospital care while delegating to others as appropriate. Educates patients and caregivers about their plan of care, transition of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, families, and all members of the interdisciplinary patient care team. Manages assignments within the care delivery model of Modified Primary Nursing. Models the tenets of Relationship Centered Care through various patient interaction actions. Considers the patient's values, preferences, cultural diversity, and expressed needs in all aspects of care. Establishes and maintains a therapeutic relationship with patients and families. Uses evidence-based practices to enhance patient care perceptions. Initiates service recovery for patients and family members who have concerns. Demonstrates caring, respect, compassion, empathy, and active listening through dialogue, body language, and actions. Implements patient-centered care, identifying barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. Engages in formal and informal peer and institutional review processes. Collaborates with interdisciplinary teams to create quality improvement projects and evidence-based practice activities. Documents nursing practice in compliance with regulatory and legal requirements. Facilitates patient throughput on a shift basis. Contributes to the clinical environment supporting colleagues, patients, and their families. Advances clinical competence in nursing practice towards expert status. Projects a professional image to colleagues and communicates with care, respect, and compassion. Incorporates ethical principles into decision-making for patients and families. Encourages a spirit of inquiry, life-long learning, and innovation for self and others. Participates in shared decision-making through nursing department committees and unit initiatives.
