Job Title: Home Health Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum of 2 years as a Registered Nurse License: Current RN license in good standing Certifications: Current automobile insurance and valid driver's license Must-Have: - Patient care management - Bedside nursing - Symptom management - Crisis intervention - Family intervention Description: The Registered Nurse reports to the Director of Professional Services. This role administers skilled nursing care to clients requiring professional services in their homes. The RN teaches and supervises the family and other members of the nursing team. These services are performed in accordance with the physicians' orders and under the direction of the Clinical Manager. Essential duties include: Administering nursing care according to the patient's plan of care and needs. Caring for multiple patients throughout the day in their individual home environments. Teaching the patient and/or family members about new diagnoses, care, and support skills. Communicating regularly with RN Case Managers, supervisors, and the interdisciplinary care team. Coordinating and delivering nursing care to patients and families. Assessing and evaluating the patient's level of care, needs, and requirements. Assessing the patient's response to the Plan of Care and recommending changes as needed. Evaluating outcomes of care. Supervising LPNs, home health aides, and paraprofessionals according to regulatory guidelines. Participating in on-call coverage as needed. Submitting work in a timely manner and updating patient medical records. Participating in interdisciplinary team meetings to coordinate care and exchange information. Applying policies and procedures of the agency and regulatory agencies in providing care. Participating in quality assessment performance improvement programs. Ensuring growth and profitability of the company through responsible use of resources. Meeting established productivity standards. Providing education to the patient/family regarding the disease process, self-care techniques, and community resources. Accepting direction from the Director of Professional Services/Patient Care Manager. Participating in community programs and committees as requested. Initiating appropriate preventive and rehabilitative nursing procedures. Preparing clinical and progress notes that demonstrate progress toward established goals. Informing the physician of changes in the patient's needs and outcomes of interventions. Assessing the ability of the caregiver to meet the patient's needs at admission and throughout care. Evaluating patient/family response to care on an ongoing basis. Communicating information using current processes and technology available. Adhering to established personnel policies. Accepting other assignments deemed appropriate. Implementing the individualized plan of care and recommending revisions as necessary. Managing discomfort and providing symptom relief and specialized nursing skills related to palliative and end-of-life care. Determining scope and frequency of services needed based on acuity and patient/family needs. Conducting initial and ongoing comprehensive assessments, including OASIS at the appropriate time. Evaluating outcomes of care.
