Travel Nurse RN - Case Manager - $2,800 to $2,850 per week in Tuba City, AZ - Position Summary * Incumbent serves as an Outpatient/Inpatient Social Worker/Registered Nurse (RN) Case Manager and is responsible for direct supervision and supportive contact for the specific group of identified high-risk patients. * The Social Worker/RN Case Manager is responsible for designing and managing a continuum of care focusing on empowering clients to achieve demonstrable outcomes and self-sufficiency. * The Social Worker/RN Case Manager is responsible for assessment, service planning, and resource acquisition, monitoring progress and initiating and responding to emerging client needs. * The Social Worker/RN Case Manager will link the client with Community Social Service providers, health care providers, substance abuse, and mental health providers to achieve specific goals. * The Social Worker/RN Case Manager is responsible for utilization review of patient cases in the inpatient and outpatient services departments, and review of medical necessity of referrals to specialty providers for services not normally provided within the facility. * The incumbent will work with a multidisciplinary team to promote the efficient and effective use of a variety of health care resources by focusing on the whole patient to provide a seamless continuum of care. * The goals are the provision of quality care, decrease the fragmentation of care, enhancement of patient self-care and quality of life, and cost containment. * The incumbent will work closely work with Providers, Nurses, Social Workers, Physicians, the PRC department, and all health care providers to ensure implementation of appropriate resources in the fulfillment of utilization goals for the hospital and third-party payors. * The Social Worker/RN Case Manager will be responsible to the Director of Case Management/Care Coordination.   Education: MSW Social Worker: * Master’s Degree in Social Work (MSW), OR   RN Case Manager: * Associates degree in Nursing, must obtain bachelor’s in nursing within 2 years of hire.   License/Certification: * Must have and maintain current Basic Life Support (BLS) certification by the American Heart Association throughout employment   MSW Social Worker: * Licensed Master Social Worker (LMSW). A valid, current, full and unrestricted Professional License to practice LMSW or LCSW in any state of the United States of America, The Commonwealth of Puerto Rico, or a territory of the United States   RN Case Manager: * A valid, current, full and unrestricted Professional Nursing License to practice nursing in any state of the United States of America, The Commonwealth of Puerto Rico, or a territory of the United States   Experience: *  Must have a minimum of three (3) years of experience in the clinical care setting *  Experience and knowledge of discharge planning, case management, or utilization review processes. *  Demonstrate knowledge of electronic health record systems. 

Travel Nurse RN - Case Manager - $2,426 to $3,070 per week in White Plains, NY - The RN Care Manager performs the initial comprehensive assessment on admission in accordance with the Care Management Department policy, screening all patients by utilizing established tools for high risk indicators to ensure high risk patient populations receive the appropriate supportive services for discharge to prevent readmission and assess all populations for potential discharge planning needs. Provides ongoing reassessment of needs throughout the hospital course. Performs UR activity per department policy.

We currently have an opportunity for two experienced RN Care Manager for a 13 week assignment at our main campus in York, Maine. These are 40 hour per week positions with variable shifts of either 8:30 to 4:30 or four ten (10) hour shifts that will start at 7:30 am - 5:00 pm . There is no call, but weekend and holiday rotation is required. The RN Care Manager will coordinate concurrent utilization management activities house-wide and is the main link between the patient, health care team, organization, and payors during the patient's hospital stay. Duties and Responsibilities: Ability to conduct an assessment of patient needs, identify resources required to transition patients to the next setting, and secure the necessary resources. Collaborate, educate, and lead an interdisciplinary team of professionals in daily care rounds/care conferences/care planning. Excellent communication skills in written, verbal, and electronic systems such as email or with software applications currently used with the hospital. Ability to work with community organizations to secure durable medical equipment, health insurance authorizations, arrange transportation. Participate in the length of stay of an inpatient hospital population. Knowledge of hospital reimbursement methods and quality measures applicable to the care management department. Education/Licensure/Certifications/Qualifications: Associate or Bachelor degree-prepared Registered Nurse with 3-5 years clinical experience in medical/surgical nursing, home care, hospital case management and/or previous utilization review experience. Must have utilization reveiw and discharge planning experience. Certification by a nationally recognized case management or utilization review organization preferred Must have active BLS. CCM or ACM preferred. Minimum Guaranteed Hours: 3
