THE POSITION: A certified medication aide is a certified nursing assistant (CNA) responsible for administering daily medication to patients in a hospital or medical facility. Also referred to as medical aide technicians, their duties include monitoring patients, reporting changes, and collecting samples. The ideal medication aide will be extremely detail-oriented and possess in-depth knowledge of patient care procedures. Travel Nurse RN - Hospice - $2,600 to $2,650 per week in Ronan, MT - _Job Summary:_ The RN – Case Manager provides skilled nursing, care, and case management for assigned patients in their place of residence; admits patients to services; coordinates and integrates services to ensure continuity of care; facilitate the achievement of quality, clinical, and cost outcomes; negotiates, procures and coordinates services and resources needed by the patient or family; creates opportunities and systems to enhance outcomes; and uses the nursing process of assessment, planning, and implementation of care, including patient/family instruction and evaluation through an interdisciplinary team approach. This position may start as a Visiting RN and move to Case Manager. The same clinical responsibilities apply to both designations. _Essential Duties and Responsibilities:_ * Provides skilled nursing and coordinates care to patients and family members in a residential setting through a team approach and based upon systematic evaluation as authorized by a physician. * Observe, assess, evaluate, and plan interventions about patients’ physical and psychosocial statuses, including changes in condition and/or reactions to treatments or medications for the duration of contact with patients. * Oversees care provided by Home Health Aides and CNAs to assigned patients and evaluates compliance with the plan of care. * Assures continuity of care through effective communication, information exchange, and case conferences with other disciplines, community health, support, and social agencies. * Provides quality care and appropriate symptom management for positive clinical outcomes and promotes optimal health and quality of life through teaching, counseling, appropriate interventions, and referrals. * Maintains accurate, current, and complete patient records by Agency standards and OASIS requirements as well as knowledge of current reimbursement rules and regulations. * Participates in team scheduling coordination and “on-call” rotation schedule as necessary to meet patient care needs by assessing and evaluating patient needs via telephone and advising or making home visits as required. * Participates in quality improvement activities and attends in-service training as assigned. * Performs other duties as assigned by supervisor.
