Travel LPN / LVN - Long-Term Care - $40 to $42 per hour in Liberty, NY - DURATION: 13 Weeks to Long-Term Contract SCHEDULE: * 3:00 PM – 11:00 PM Shift * 11:00 PM – 7:00 AM Shift * Every other weekend required WORK ARRANGEMENT: On-Site POSITION SUMMARY: The Licensed Practical Nurse (LPN) provides professional nursing care to residents in accordance with established care plans and physician orders. The LPN works under the supervision of a Registered Professional Nurse and is responsible for delivering quality resident care, administering treatments and medications, monitoring patient conditions, and maintaining accurate documentation. The role includes supervising Nursing Assistants and clerical staff assigned to the nursing unit while exercising appropriate clinical judgment within established guidelines. KEY RESPONSIBILITIES: * Provide nursing care according to residents’ individualized plans of care. * Administer medications and therapeutic treatments as prescribed by physicians. * Monitor and record vital signs and resident health conditions. * Collect specimens and assist with diagnostic procedures. * Follow established care plans and communicate changes in resident conditions. * Maintain accurate medical records and nursing documentation. * Communicate effectively with physicians, registered nurses, residents, and families. * Assist with resident admissions, transfers, and discharges. * Supervise and provide guidance to Nursing Assistants and assigned support staff. * Ensure compliance with facility policies and nursing standards. WORK ENVIRONMENT: * On-site healthcare facility setting. * Provides direct care to residents requiring ongoing nursing support. * Requires collaboration with nursing staff, physicians, residents, and support personnel.

Travel LPN / LVN - Long-Term Care - $35 to $40 per hour in Omaha, NE - Duration: 3 months (With possible extension) Schedule: * 7 AM - 3:30 PM * 3 PM - 11:30 PM * 11 PM - 7:30 AM Job Responsibilities:  * Perform direct hands-on care in accordance with the plan of care, physician orders, and nursing standards of practice while utilizing the principles of nursing and basic physical assessment skills. * Provide patient/resident teaching in accordance with their needs and standards of care. * Accurately document assessments, care, teaching, and observations in the medical record., including, but not limited to, the medical needs of the individual, care provided, date and time of care, and response to care; ensure entries reflect the care plan being followed and update as needed. * Participate in developing, implementing, evaluating, and updating individualized interdisciplinary care plans to ensure care/medical needs are being met. * Direct and monitor Medication Aides including responsibility for observing and taking appropriate action regarding any desired effects, side effects, interactions, and contraindications associated with the medication(s) administered by a Medication Aide. * Accompany physician on rounds and assist physician with exams and treatments as needed. * Assist with orienting, training, evaluating, and monitoring the work of subordinates and team members, and assure adherence to policy/procedures, work rules, and state and federal laws/regulations. * Work cooperatively and respectfully with team members, families, visitors, and volunteers to create a positive and pleasant environment. * Complete nursing rounds, assist/consult with nursing/medical staff, inform team

Travel LPN / LVN - Long-Term Care - $35 to $40 per hour in Vestal, NY - Duration:  06 months to long-term Shift: 6:45 AM - 7:15 PM,  6:45 PM - 7:15 AM, 2:45 PM - 11:15 PM or 11:45 PM - 7:15 AM             Job Description: * Assists new residents in adjusting to unit, philosophy and goals; * Report to Charge Nurse any accident or change of condition of resident and documents on progress notes in chart; * Discusses clinical issues regarding resident care with Charge Nurse; * Works as a member of the interdisciplinary team to Assist with preparation, implementation and continuing evaluation of the care plan; * Observes, records and reports progress or any change in resident's condition; * Updates the care plan as needed; * Carries out therapeutic treatments; * Administers and dispenses medications (with exception of I.V.) to residents, observing for side effects and effectiveness; * Secures/accounts for control drugs; * Maintains nursing care and administrative records and reports; * Attends staff meetings and in-service programs.

Travel LPN / LVN - Psych - $40 to $42 per hour in Warm Springs, MT - Duration: 13 weeks to long-term Contract Schedule: 7 am-7 pm and  7 pm-7 am. Every other weekend Responsibilities: * Record a patient's medical history accurately. * Take and record measurements of blood pressure, temperature, heart rate etc. * Observe patients under treatment to identify progress, side-effects of medications etc. * Monitor patients' condition including fluid intake and output and compose patient charts. * Assist ailing patients in daily necessary activities such as eating, bathing etc. * Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks. * Collect samples for lab testing as assigned. * Provide emotional and psychological support when needed. * Communicate with patients' family or friends to provide advice, comfort and release instructions.

Travel LPN / LVN in Fort Dodge, KS - Duration: 3 Months to long-term Shift: 8 hours; 1st, 2nd, and 3rd Per diem Job Responsibilities: * The Licensed Practical Nurse must be able to demonstrate the knowledge of skills necessary to provide care appropriate to the patients served. * The individual must demonstrate knowledge of the principals of growth and development and possess the ability to assess and interpret data to identify age specific needs. * Must provide leadership and coordination of quality patient care by fostering therapeutic relationships, active treatment and supporting the hospital in its mission and vision. Provides patient care and recognition of behavioral and physical changes. Carries out nursing care recommendations and orders. * Maintains a therapeutic environment. Passes medications and oversees the medication room. * Serves as shift leader if assigned, making assignments to direct care staff, and providing oversight to subordinate nursing staff working the unit.
