Although a week is barely enough time to start expressing our gratitude for all of the amazing nurses out there, Travel Nurse Source is happy to announce our 2017 National Nurses Week Celebration Sweepstakes!

We’ve been honored to help nursing professionals and our amazing clients fill positions with only the best candidates in the industry — now we’d like to give back!

The ANA’s theme for this year’s celebration is The Balance of Mind, Body, and Spirit — something absolutely essential to maintaining personal health and growth in the profession! Although, because it’s easier said than done, we’re giving away a Charge HR 2 Fitbit to help one lucky nurse optimize their personal wellbeing!

To enter, participants must be at least 18 years old, a resident of the United States (including the District of Columbia and Puerto Rico), and working as an RN, LPN, or CNA. Simply “like” or leave a Facebook comment for this video telling us how you stay balanced as a nurse. It can be short and sweet, or as detailed as is useful — just be sure to keep HIPPA in mind and use your best judgment in what you share! Also, feel free to share this video or tag a friend, however, this is not required in order to enter.

2017 National Nurses Week Celebration Sweepstakes | Official Rules

1. Eligibility

The Travel Nurse Source 2017 National Nurses Week Celebration Sweepstakes (the “Sweepstakes”) is open only to legal residents of the fifty (50) United States (including the District of Columbia and Puerto Rico) who are at least eighteen (18) years old and have reached the age of majority in their state or territory of residence at the time of entry (19 in Alabama and Nebraska; 21 in Mississippi). The Sweepstakes is subject to all applicable federal, state, municipal, territorial, and local laws and regulations and is void where prohibited by law. (NEW JERSEY NOTICE: This giveaway is not void in the state of New Jersey.) Participation constitutes participant’s full and unconditional agreement to these Official Rules (“Official Rules”) and Sponsor’s decisions and interpretations of these Official Rules, which are final and binding in all matters related to the Sweepstakes. Winning a prize is contingent upon fulfilling all requirements set forth herein. The Sweepstakes is in no way sponsored, endorsed, approved or administered by or associated with, Facebook and/or Fitbit, Inc.

2. Sponsor:

This Sweepstakes is sponsored by Track5Media, LLC, 941 Wheatland Avenue, Suite 402 Lancaster, PA 17603, U.S.A. (“Sponsor”)

3. Timing:

Entries must be submitted between 8:00 AM Eastern Time (“ET”) on May 1, 2017 and 11:59 AM ET on May 12, 2017, (“Sweepstakes Period”). Sponsor’s computer, or that of its designee, shall serve as the official timekeeper for the Sweepstakes.

4. How to Enter:

Shortly before the start of the Sweepstakes Period, Sponsor will post a video to Travel Nurse Source’s Facebook Page along with instructions on how to enter the Sweepstakes. To enter:

Leave a comment or “like” the video post.

Limit one (1) entry per person. Entries must be received during the Sweepstakes Period to be eligible. Entries must also comply with the “Entry Requirements” described below. The use of automated, robotic, macro, programmed, or what could be described as quick-entry programs is prohibited and will result in disqualification. In case of dispute, entries will be declared by the authorized account holder of the Facebook profile associated with the natural person who is assigned to an e-mail address by an Internet Access Provider, on-line service provider, or other organization (e.g. business, educational institution, etc.) that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address. Sponsor will not verify receipt of entries. Sharing the post or solely tagging friends does not indicate entry to the Sweepstakes and will not increase the chance of winning. Liking Travel Nurse Source’s Facebook page is not required.

5. Entry Requirements:

Entry can be completed by leaving a comment or “liking” the Facebook video post hosting Travel Nurse Source’s Official 2017 National Nursing Week Celebration Sweepstakes.

Participants must be currently be employed as a Registered Nurse (RN), Licensed Practical Nurse (LPN), or Certified Nursing Assistant (CNA). Verification of this requirement is at the sole discretion of Sponsor and will be confirmed during the attempted notification of the potential winner.

Ineligible entries include entries that fail to comply with these Official Rules, as well as those that are threatening, abusive, harassing, defamatory, libelous, hateful, embarrassing to another person or entity, deceptive, obscene, invasive of another’s privacy, tortious, contain racial slurs, or contain explicit or graphic descriptions or accounts of, sexual acts, go against HIPPA rules, or are not within the appropriate spirit of the Sweepstakes, as determined by Sponsor in Sponsor’s sole discretion (“Entry Requirements”).

6. Winner Selection / Notification:

A drawing will be held on or around 12:00 PM ET on May 12, 2017 randomly selecting one (1) potential winner. Following this drawing, the potential winner will be notified using a private message via Facebook and subsequently must submit a mailing address to receive prizes as instructed by Sponsor. Except where prohibited, the potential winner may be required to sign and submit a Declaration of Compliance and Liability and Publicity Release (“Declaration”) which must be received by Sponsor and/or Sponsor’s agent within Seven (7) days of the date the notification or attempted notification is issued in order to claim his/her prize. If a potential winner fails to submit a mailing address as instructed within a twenty-four (24) hour period from when the notification is sent, cannot be contacted, or fails to sign and return the Declaration within the required time period, or fails to otherwise comply with these Official Rules, the potential winner forfeits the prize. Time permitting, an alternate potential winner will be randomly selected. All requirements stated in these Official Rules will be enforced.

Potential winner is subject to verification by Sponsor, whose decisions are final and binding in all matters related to the Sweepstakes.

7. Sweepstakes Affiliations:

The winner will receive one Charge 2 HR Fitbit. The gifting of this prize is not in any part endorsed by and/or sponsored by Fitbit, Inc. Additionally, this Sweepstakes is not affiliated with, Fitbit, Inc and/or their affiliates.

Furthermore, the gifting of this prize is not in any part endorsed by and/or sponsored by the American Nurses Association (ANA). Additionally, this Sweepstakes is not affiliated with, ANA and/or their affiliates.

Finally, this Sweepstakes is in no way endorsed by and/or sponsored by Facebook and their affiliates.

All costs and expenses associated with the acceptance and use of the prize not specified herein as being awarded, including, but not limited to, all federal, state and local taxes and other miscellaneous expenses are the winner’s sole responsibility. The prize will be awarded “as is” with no warranty or guarantee, either expressed or implied (including, without limitation, quality, merchantability, and fitness for a particular purpose) in connection with this Sweepstakes or any prize. Prize color/specification to be determined by Sponsor’s discretion and no transfer, assignment or substitution of prize by winner is allowed. Sponsor also reserves the right to substitute a prize with one of equal or greater value if the designated prize should become unavailable. More than the advertised number of prizes will not be awarded under any circumstances. POTENTIAL WINNER IS SUBJECT TO VERIFICATION BY SPONSOR, WHOSE DECISIONS ARE FINAL AND BINDING IN ALL MATTERS RELATED TO THE SWEEPSTAKES.  A PARTICIPANT IS NOT A WINNER OF ANY PRIZE, EVEN IF THE SWEEPSTAKES SHOULD SO INDICATE, UNLESS AND UNTIL PARTICIPANT’S ELIGIBILITY (WHICH MEANS FULL COMPLIANCE WITH THE TERMS AND CONDITIONS OF THESE OFFICIAL RULES) HAS BEEN VERIFIED AND PARTICIPANT HAS BEEN NOTIFIED THAT VERIFICATION IS COMPLETE.    

8. General Conditions:

Sponsor reserves the right to cancel, suspend and/or modify the Sweepstakes, or any part of it, if fraud, technical failures, virus, bugs, human error or any unforeseen factor impairs the integrity, availability, fairness, or proper functioning of the Sweepstakes. Sponsor reserves the right to disqualify individuals found to be tampering with or undermining the operation of the Sweepstakes, or those acting in violation of the Official Rules. Sponsor reserves the right to seek damages from any such person to the fullest extent permitted by law. Failure to enforce any term of these Official Rules shall not constitute a waiver of these provisions.

Proof of transmission (comment or like via Facebook) does not constitute proof of entry beyond Sponsor’s record of entry.

All federal, state and local laws and regulations apply. Issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of entrants and Sponsor in connection with the Sweepstakes, shall be governed by, and construed in accordance with Pennsylvania law without giving effect to any choice of law or conflict of laws.

No purchase of any products or services provided by Track5Media, LLC is necessary for entry and will not increase chances of winning.
